This form should only be used to report your child as absent due to illness.

Parents and carers are required to contact the school for every day before 9am of their child's absence to ensure our safeguarding responsibilities are adhered to.

Please note that we stipulate that a child must not return to school until 48 hours after the last bout of any sickness or diarrhoea to prevent the spread to other members of the school community.

If your child has a high temperature/fever please wait for this to go back to a normal temperature before returning to school.
